Are your friends telling you that you look tired or fatigued? Do you have dark circles under your eyes? Or perhaps you have a difficult time putting on upper eyelid makeup because of the excess skin. Maybe you are getting forehead headaches or wrinkles because you elevate your eyebrows to see better. Then you should consider eyelid or blepharoplasty surgery.

“Bleph” comes from the Greek word for eyelid. “Plasty” is to shape or mold in Greek. In blepharoplasty surgery, the plastic surgeon removes the excess skin and fat to rejuvenate the eyes.

Upper eyelid blepharoplasty is a wonderful procedure that in 45 minutes to one hour can make an incredible difference in your appearance. The incision is made in a natural crease and heals within one week. The excess skin and fat is removed. There is no pain and many patients return to work or light activities in two to three days.

Lower eyelid blepharoplasty can be performed through a skin (transcutaneous) approach or through a hidden approach inside the lower eyelid (transconjunctival). Our physicians will discuss the best approach for you during your consultation.

The transcutaneous or external approach requires an incision under the lower lid eyelashes. The advantage is removal of both excess fat and skin. The incision usually heals within one week but any residual redness may be covered with makeup after one week.

The transconjunctival or inside approach hides the incision inside the lower eyelid. Only the excess fatty bulges are removed with this approach. Many times the physicians at the Mandell-Brown Plastic Surgery Center will utilize laser resurfacing to remove the lower eyelid creases and crows feet at the same time.

The lower eyelid surgery usually produces some bruising that resolves quickly. Following our instruction will significantly reduce the bruising.
With lower eyelid surgery there may be some discomfort, but this lasts only a few days.

Patients with dark circles under their eyes may benefit not only from lower eyelid surgery to remove the fatty bulges, but they may require some bleaching creams or peeling agents to lighten the skin pigment. This will be discussed during your consultation.

For those patients with extremely heavy upper eyelid skin, insurance coverage may help with the cost of upper eyelid blepharoplasty. If visual field is limited, insurance carriers may provide coverage. This can be determined at the time of your consultation and verified by an independent eye exam.

FAQs

Q. I am 24 years old and I can not put on make-up on my upper eyelids. Am I too young for eyelid surgery?

A. There is no set age for eyelid surgery. Need for surgery is based on physician assessment and patient needs. What determines excess skin and fat in the eyelid area are:
1. Heredity
2. Age
3. Gravity
4. Predisposing medical conditions
If you truly have excess skin, upper eyelid surgery can make a dramatic improvement.

Q. Some of my friends have had eyelid surgery and it is easy to tell. They looked startled or surprised. How does this happen?

A. Some surgeons are extremely aggressive or prefer an “overdone” look. At the Mandell-Brown Plastic Surgery Center, we strive for a natural look. With conservative surgery, the patient should look improved and natural. It is so important to know your plastic surgeon’s philosophy. Be sure to see pictures of other patients. If you don’t like what they show you, chances are you won’t be happy with them. If they refuse to show samples of their work—beware!

Q. What anesthesia is required for eyelid surgery?

A. We usually provide oral relaxation with medication such as Valium. Local anesthetic is then injected in the upper eyelid skin. The patients do great with this, but someone should drive the patient home. For lower eyelid surgery, we utilize intravenous sedation to relax the patient in a deeper state. For transconjunctival surgery in which the incision is made on the inside of the eyelid, anesthesia personnel are utilized to provide even deeper relaxation. General Anesthesia in which the patient is “put to sleep” is not required.

Q. Do men have eyelid surgery?

A. Approximately 25% of our blepharoplasty surgery are male. Often they have heavy brows that also contribute to the upper eyelid excess. See section on BROW LIFT.

Q. If I have lots of wrinkles under my eyes will the eyelid surgery help eliminate them?

A. The eyelid surgery removes excess fat and skin. Wrinkles are better treated with facial peels, lasers, or Botox.
Botox helps the wrinkles when smiling and wrinkling the eyelids. The peels and lasers help eliminate the lines that are present when your face is relaxed. The physicians will discuss this at the time of your consultation.

Preoperative Preparation

Congratulations! You have decided to have eyelid surgery. How do you prepare for your procedure?
Before the surgery, the Mandell-Brown Plastic Surgery Center will send you a list of items to get and medicines to avoid. The following should help.

Two weeks before surgery avoid the following:
1. Aspirin Products-
affects platelets and leads to more bruising

2. Advil, Motrin, etc…-
may affect platelets and can possibly cause bruising

3. Vitamin E-
studies show this vitamin can affect platelets

4. Herbal Products-
many of the natural occurring herbal items can affect platelets and bleeding factors, so stop 2 weeks before.

For example:
Ginseng-
can be associated with increased blood pressure and heart rate. It may also increase bleeding.
St. John’s Wort-
may prolong effects of anesthetic agents
Echinacea-
may reduce your immune system
Feverfew-
may increase bleeding
Garlic-
lowers blood pressure but may increase bleeding
Valerian-
may prolong anesthetic agents

One week before surgery start the following:

  1. Dr. Mandell-Brown’s office should have sent a vitamin packet. Start taking Vitamin B and C each day for a week before and then continue for a week after the surgery. These two vitamins help the tissues heal after surgery. Do not take more than the one
    vitamin each day.
  2. Purchase the following items to be used after surgery:
    • Peroxide or Witch Hazel (determined during consultaion)
    • Q tips
    • Johnsons Baby Shampoo
  3. Purchase your prescriptions from the pharmacy.
  4. Make sure you have your history and physical completed along with any recommended lab work.
  5. Relax the night before surgery. Do not eat or drink anything after midnight.
  6. The morning of surgery thoroughly wash your face and hair before arriving at
    The Mandell-Brown Plastic Surgery Center. Remember, no coffee or other fluids.
